Current Student Forms

You will need to log-in to your CCC student account to access these forms.  Completed forms will route to the Registration & Enrollment Services Department for processing; you will be contacted at your CCC student email address (@students.91src.com) within one week with confirmation or any next steps you may need to take.

Registration Add/Drop form - this is to be used during instructor permission registration period 8/30 - 9/5

*Any registration during this time will require instructors permission to add to the course.

If you are attempting to add, drop, or withdraw from a course(s), please log in to myCCC and choose the "registration" icon.  Step-by-step instructions for using self-service registration can be found on our Student How-To Guides page.

Academic 
  • Request for Evaluation
    • Submit a professional license/certification OR
    • Submit specialized college credit for transfer credit evaluation
  •    Comet Scholars Program Interest Form
    • Sign-up for a schedule of fully online Associate of Business courses OR
    • Sign-up for a schedule of fully in-person Associate of Arts: General Studies courses

Personal / Residency / PDF Forms

Due to the content and associated documentation, these forms are required to be submitted in-person, via secure upload or emailed. Signatures are required.

Digitally Signed Forms: PDF forms must be downloaded, opened, and signed in Adobe Reader and uploaded using our Secure Upload site
